Request for Accommodations




Documentation
Documentation is required to receive accommodations in relation to a disability. While it is not required that documentation meet all of the guidelines below, these guidelines may provide an example for what we typically look for within documentation.

1. The diagnosis is clearly stated
2. Information is up-to-date
3. Relevant educational, developmental, and medical history is presented
4. The diagnosis is supported with test results, observations, symptoms, or other means to diagnosing specific conditions
5. The functional limitations and its impact on the student's academic experience is detailed
6. Accommodations are recommended and justified
7. Evaluators' professional credentials are established


If applicable, please upload your 504 Plan or Individualized Education Program (IEP).

If you do not have your documentation readily available to upload, documentation can be submitted the following ways:

At this time, accommodations in relation to a disability are only available to students with a current medical or disability diagnosis. If you are interested in obtaining a diagnosis, please contact your medical provider. The Office of Personal Counseling as well as Health and Wellness may be able to provide a referral to a provider if needed. Personal Counseling can be reached at 314-529-9556.
