You should only fill out this form if you are an ESL student who has received a letter from the Division of Student Success and you would like to use Extended Time (Time-and-a-half, 150%) on an exam. Remember, you should talk to your instructor before requesting Extended Time on your tests or quizzes.

Important information, please read!

Requests should be made 3 days in advance of your test time, to allow your instructor and the Division of Student Success adequate time to coordinate your exam. If a request is made with less than 3 days notice before the test time, you may be contacted to reschedule. Certain exceptions will be made to the 3 day requirement if an earlier day/time has been coordinated with your professor and you have approved it with the testing center.

Students should contact their instructor directly to receive make-up test approval and to coordinate an appropriate date and time. All make-up tests should be approved with your professor prior to submitting this form. All make up tests are proctored in person at the Division of Student Success inside the University Library.

Option 1: The rest of the class is required to test in person, so I will take this test in person within the Division of Student Success.Option 2: The rest of the class is able to take this test virtually over Zoom, so I am taking this test over Zoom and need to be proctored over Zoom by the Division of Student Success. I understand I will receive an email with further instructions.Option 3: The rest of the class is taking the test on their own unproctored, so I am taking the test on Canvas on my own time outside of class. I am submitting this request so my instructor knows to extend my time.

Important Information

Please do not submit this request unless you have received approval from your professor.

Upon submission of this form, your instructor will receive a copy of your request. An appointment will be made for your selected date and time. You will not receive an additional confirmation, as your test date has been confirmed with the submission of this form.

Students who use the testing center for make up testing are expected to abide by all testing center policies, as well as uphold all academic integrity policies. If a suspected policy violation occurs, students will be required to meet with the testing center director to discuss their access to the testing center in the future.
