Instructions: Consistent with current CDC guidelines, employees who are
fully vaccinated are no longer required to quarantine after domestic or
international travel. To help the university adhere to these guidelines and
manage quarantine guidelines, all employees must complete this form to help
Human Resources determine if quarantine after travel is required and notify
your supervisor so they can discuss with you. Employees requesting paid time
off during the travel period should also concurrently submit a request in ADP.
Supervisors will review and evaluate requests for paid time-off and
travel based on operational needs as they normally would.
